Student Services
At Deanmore Primary School we value the partnership fand support of External Providers (e.g. Speech Therapists, Occupational Therapists). If you would like your child to access external support during school time in 2025, please complete the following forms:
The PISP form will be completed and needs to be sent in first. If and when this is approved, you will be asked to complete a Service Schedule form. It is important that you include all relevant information on this form including:
- Preferred days and times
- Attachments form your provider - Insurance papers, Working with Children form, Education Police check, NDIS number, goals for your child (Please note - If your provider has a NDIS numer, they do not need an Education Police check).
- Signatures from yourself and your external provider
The forms will be reviewed and a time allocated abased on availability of space, class commitments and the needs of your child. As a part of the ongoing process, your external provider will need to regularly update your child's goals and report progress to both you and the school.
Please note sessions will not start until Term 1 Week 3 Monday 17th of February once all the paperwork has been processed. Sessions will end in Term 4 Week 9 Friday the 12th of December, unless otherwise noted on the forms.
